Clinic Regulations
1. Service registration at Got Ur Back is equivalent to acknowledgment and acceptance of these Regulations.
2. Each new patient appointment at Got Ur Back involves creating a user account in the external online booking system Booksy and in the medical documentation software Zarejestrowani, both of which comply with all regulations under the RODO. Account creation in these services is essential for effective management of the reservation system and patient communication.
3. Appointments at Got Ur Back can be made:
- In person, by visiting Got Ur Back (Słowackiego 51/4, Katowice) during working hours.
- By phone, at +48 508 852 486 during Got Ur Back working hours.
- Online via the Booksy appointment booking system, available 24/7 (apart from emergency situations).
4. The clinic operates on Mondays, Wednesdays, and Thursdays from 12:00 PM to 8:00 PM and Fridays from 9:00 AM to 5:00 PM. In exceptional situations, the clinic may not be open during these hours or days. The clinic is closed on Saturdays, Sundays, and public holidays.
5. Patients booking an appointment are required to provide necessary personal and health information during registration.
6. All patients must familiarize themselves with the guidelines for preparing for their first visit, as well as the RODO documentation.
7. In case of lateness, the appointment duration will be reduced by the delay time. Being more than 15 minutes late without prior notice is treated as a cancellation. In this case, the specialist reserves the right to attend to the next patient.
8. Individuals causing damage to the facility will be held financially responsible for the damages. Parents or legal guardians are financially responsible for damages caused by minors.
9. Smoking is strictly prohibited on clinic premises.
10. Got Ur Back is not responsible for personal belongings left on the premises. Lost items will be stored for up to one month, after which they will be discarded.
11. Patients have the right to:
- Receive full information about their health status and proposed treatment methods.
- Have their dignity and privacy respected.
- Submit complaints and suggestions regarding the services provided.
12. Patients are obliged to:
- Adhere to scheduled appointment times.
- Inform clinic staff about changes in their health status.
- Behave in a manner that does not disrupt the clinic’s operations or compromise the comfort of other patients.
13. Got Ur Back staff are obliged to:
- Uphold professional ethics and comply with applicable laws.
- Provide patients with high-quality medical services.
- Respect patient privacy and maintain confidentiality.
14. Got Ur Back staff have the right to:
- Receive necessary information from patients to deliver services according to established standards.
- Decline service provision to a patient.
15. Complaints can be submitted via email (pawmat93@gmail.com). Got Ur Back is committed to addressing complaints within 14 business days and providing a response to the patient.
16. Matters not regulated by these Regulations shall be governed by the provisions of the Civil Code.
Cancellation Policy
If you are unable to attend your appointment, you must cancel the reservation by phone or SMS exclusively at +48 508 852 486, or via the Booksy app, no later than 24 hours before the scheduled visit.
Canceling appointments through social media (Instagram, Facebook, Google) or via WhatsApp will not be accepted.
If you fail to attend your appointment without prior cancellation or cancel less than 24 hours before the scheduled visit, Got Ur Back reserves the right to impose an administrative fee equal to 50% of the appointment’s cost.
A significant delay in arriving for your scheduled appointment may also be treated as a no-show. Whether the administrative fee will be applied in such cases is at the discretion of the specialist.
The administrative fee is only applicable in cases of non-compliance with the Cancellation Policy. If you attend on time or cancel your appointment no later than 24 hours before the scheduled time, Got Ur Back will not impose the administrative fee.
